Liability Release/Waiver Form

Participant Waiver

I represent and warrant that I am in good physical health and do not suffer from any medical condition that would limit my participation in the classes offered at Bend+Fly West End. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any of the yoga classes, programs, or workshops. I understand the risks associated with the activities offered by Bend+Fly West End and I agree to follow all instructions so that I may safely participate in classes, workshops, or other activities.

I hereby WAIVE AND RELEASE Bend+Fly West End, its owners, officers, employees, and instructors from any claim, demand, cause of action of any kind resulting from or related to my participation in the programs offered at the facility. In taking part in AntiGravity classes, yoga classes, workshops, or other activities whilst on the premises or car parks at Bend+Fly West End, I understand and acknowledge that I am fully responsible for any and all risks, injuries, or damages, known or unknown, which might occur as a result of my participation in the classes, workshops, or other activities.

Use of Likeness

I grant my permission to Bend+Fly West End to utilise any photographs, motion pictures, videotapes, recordings, and other references or records of the activity which may depict, record or refer to me for any purpose (“Likeness”), including commercial use by the released parties, their sponsors and their licensees. I understand and agree that I will not be compensated or receive additional consideration for consenting to the use of my Likeness and that I will not be given a chance to receive, inspect or approve the promotional or marketing material, messages and/or content that may use my Likeness.

Child (Minor) Release & Consent

Child Release

In consideration of participation in a fitness activity, I agree, on behalf of the above-named child, his/her heirs and representatives, to fully and forever release Bend+Fly West End, its officers, volunteers, agents and employees from any and all liability, claims, demands, damages, actions, or causes of action whatsoever arising out of or related to belonging to my child or me, related to the activity, regardless of cause. This release covers everything that happens from the time I leave my child at Bend+Fly until I pick them up.

Child Consent

To the best of my knowledge, the above-named child can fully participate in exercise. I am aware of risks and hazards connected with exercise and my child hereby elects to voluntarily participate in exercise activities, knowing that the exercise and equipment may be dangerous to my child. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury that may be sustained by my child or any loss or damage to property owned by me or my child as a result of being engaged in exercise activities at Bend+Fly West End, regardless of who caused the incident.
